The following 2 forms will help us understand your unique needs and health history. To save time please review, fill out, and sign these forms before your first visit to save time and bring it with you at your first appointment. Alternatively, you can also arrive 5-10 minutes before your first appointment to fill out these forms in our office.

Optional: For your first visit, please also bring a list of any supplements or medications you are taking regularly, and copies of any relevant laboratory or imaging results.

Helpful hints for your appointments:

Please take time to fill out the forms above prior to your initial appointment.
Please wear loose fitting clothes if possible, or bring a change of comfortable clothes with you. We might have to access points on the abdomen, arms and legs.
Make sure that you have had something light to eat before your appointment. Please refrain from highly caffeinated drinks an hour prior to your appointment time.
Please refrain from wearing perfume/cologne/scented lotions on the days of your treatment. Products with fragrances are recognized as being respiratory irritate and we have patients who are highly allergic to them. Thank you for your understanding and cooperation.
